All FOIA requests must be submitted to the FMSHRC FOIA Office at the contact information provided below.  To ensure prompt handling and processing of your FOIA request, please use the FOIA Request Form provided below and email or fax your form to the email address and fax number listed below.  

Alternatively, requesters may submit a request through the government wide FOIA request portal.
